You can receive one service hour by working with your child and filling out this form in complete sentences. The form must be completed by a parent/guardian in English or Spanish and signed. You can fill out this form after working with your child on any school-related homework assignment or project and after taking your child to visit any educational institution (college, museum, etc.). You should use the back of the page if you need more room.
