To Ánimo Venice Parents, Students, and Supporters:
This Friday, September 18, we hope to kick off a monthly tradition of community service. We are hoping to take the entire school body on a beach run where they will conduct a simple act of kindness by helping clean up Venice Beach. Students who are participating will leave school at 2:30pm and proceed for a short run to the heart of Venice Beach.
We will do beach clean-up from 3:00 – 3:25 at which time we will excuse students at the beach. Students who wish to take the bus home will run or walk back to campus in time for the late bus run at 4:30pm.
We want to invite parents and family to join us on this special event. Feel free to join us at 2:30pm on campus for the run or feel free to join us at Venice beach at 3:00pm.
The
Run -- Distance: 1.12 miles
· Run south on California Ave.
· Cross Abbot Kinney
· Go right and quick left on Andalusia
· Go south on Andalusia until Venice Way
· Right on Venice Way until we hit the beach
· Proceed to Windward and Ocean Front Walk

(I)(We), the

declare that Ánimo Venice Charter High School, it’s officials, agents and
representatives, or any individual involved in connection with this special
event shall not be held in any way for any bodily injury or harm, property
damage incurred to my child.
(I)(We), do
hereby authorize Ánimo Venice Charter High School as agents for the undersigned
to consent to any x-ray examination, anesthetic, medical or surgical diagnosis
or treatment and hospital care which is deemed advisable by and is to be
rendered under the provision of the Medical Practice Act on the medical staff
of license hospital, whether such diagnosis or treatment is rendered at the
office of said physician or at said hospital.
It is understood
that this authorization is given in advance of any specific diagnosis,
treatment of hospital are being required but is given to provide authority and
powers on the part of our aforesaid agent(s) to given specific consent to any
all such diagnosis, treatment or hospital care which the aforementioned
physician is the exercise of his best judgment may deem advisable.
This
authorization is given pursuant to the provisions of Section 28.8 of the Code
of California.
(I)(We),
understand that this special event is under the direct supervision of the staff
personnel.
